ABOUT THIS ROLE
<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>The Stroke Coordinator and Case Management Nurse (RN) has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Functions as a liaison and is responsible for planning and coordinating pathology, recognition, treatment, and post treatment care education to patients, family, caregivers, and interdisciplinary team. Provides training and performance variance advise to promote compliance with processes integral to providing quality care and in alignment with the stroke program, internal and external regulatory and certification bodies, national directives, and local facility level policy and procedures. The Stroke Coordinator & Case Management Nurse (RN) assists with patient management, care coordination, and discharge/ disposition planning for inpatient and outpatient settings. Coordinates care with multiple providers across all levels and sites of care.
