ABOUT THIS ROLE

<p>The VANJ Healthcare system is currently recruiting for one outpatient Registered Nurse to serve within our Spinal Cord Injury/ Disorder (SCI/D) Service Home Care (HC) Program. The SCI/D Home Care Program is a special PACT program designed for Veterans living with SCI/D, Multiple Sclerosis, and ALS which impact their health and functioning on a daily basis. Ongoing visits are made, in person or by video to home technology as clinically appropriate.</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care. Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the SCI-D, MS and ALS Homecare Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Plan and deliver technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Makes home visits to Veterans in the program. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
