<p>The Care Coordination Integrated Case Management (CCICM) RN Co-Coordinator is an administrative position that works in partnership with the CCICM Social Work Co-Coordinator to effectively ensure that facility nurses and Social Workers provide case management and care coordinator adhering to the CCICM framework.</p><h3>Major Duties</h3><p>List of major duties: The VA medical facility CCICM Nurse Coordinator and Social Work Coordinator serve as the VA medical facility CCICM Co-Coordinators. The CCICM Co-Coordinators are responsible for: Serving as consultants and subject matter experts on implementation of the CCICM framework, including VA and VHA policies, training, and regulations that affect care delivery to Service members, Veterans, their families, and caregivers. Coordinating and collaborating with internal and external partners to identify promising practices and gaps in services and identifying opportunities to enhance communication, collaboration, and coordination across the care continuum. Identifying and monitoring CCICM metrics and facilitating an annual review of the CCICM framework and preparing program and performance metric reports for VA medical facility leadership. Facilitating and coordinating trainings on the CCICM framework across the health care system. Ensuring Lead Coordinator assignments in PCMM are accurate per local facility guidance. Knowledge and understanding of VA programs, current policies, and procedures. Minimum of two (2) years of care coordination and/or case management experience is strongly recommended. Management experience is preferred. Care coordination or case management certification by exam is recommended. Experience with lean process improvement concepts and change management. Knowledge of adult learning concepts, teaching and/or public presentation experience. Ability to be sensitive to the needs, concerns, and issues of Veterans, families, and caregivers. Effective verbal, written and interpersonal skills. Strong analytical and problem-solving skills. Sound administrative skills and judgment and an ability to work independently and manage time effectively. Experience with risk stratification, predictive analytics, and data analysis. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
