<p>The VA Health System, has 3 vacancies for a full-time Registered Nurse with Social Work Service, Inpatient Case Management Section. The RN will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the Veterans Affairs Health Care System.</p><h3>Major Duties</h3><p>The Registered Nurse in this position functions as Case Manager for DVAMC Inpatient Medicine Teams. Inpatient Case Management falls under Social Work Service. The Registered Nurse is responsible for coordination of case management services for Veteran admitted to inpatient medicine units at the Durham VA Medical Center, working in collaboration as core member of the interdisciplinary team to coordinate safe discharge planning. The Registered Nurse Case Manager is responsible to: Identify individuals and groups of Veterans in need of care management services. Assess patients to identify needs, issues, resources and care goals, identifies resources and critical factors for achieving desired outcomes for discharge, post hospitalization recovery and/or health maintenance/improvement. Investigate complex patient care problems and promptly seek consultation and/or mobilize resources to meet care needs. Assess critical factors and/or barriers to care for goal achievement across the continuum of care services within and between organizational entities. Evaluates outcomes and documents and reports findings as needed. Assume a leadership role in utilizing problem-solving methodologies in system and/or process problems that impact patient care, staff performance, and/or system improvements. Displays leadership in identifying and evaluating issues and implementing processes to improve clinical programs and administrative systems. Analyze ethical/care issues, apply appropriate theoretical principles and ensure application of institutional policy. Initiate referrals based upon the patients' healthcare needs, eligibility, and resources available. Complies with use and maintenance of RME devices according to VA Policy and follows standard operating procedures in cleaning and/or reprocessing reusable medical equipment. Preferred Qualifications Minimum of 2 years successful practice as a Registered Nurse Prior evidence of working with the Veteran Population Prior experience is case management Demonstrated excellence in written/verbal communication, time management and working with an interdisciplinary team Specific experience as a nurse case manager. Knowledge of VA resources and program/experience working within VA system. Years of nursing experience. Evidence of certification and special knowledge obtained toward nurse case management.
