<p>The North Florida/South Georgia VA Healthcare System's Social Work Service is seeking an experienced and dynamic Registered Nurse Outpatient Caregiver Support Program Clinical Assessor. Our ideal applicants are those who want to contribute to the achievement of quality care standards, as well as the growth and development of a VA academic hospital program. Preferred experience with 1-2 years case management in a primary care/ mental health setting.</p><h3>Major Duties</h3><p>The Caregiver Support Program Clinical Assessor Nurse is responsible for knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day to day operation of the program, and all reporting requirements within the assigned Veterans Integrated Systems Network. Delivers fundamental knowledge-based care and counseling to assigned clients while developing technical skills. Duties include but are not limited to; Executes position responsibilities that demonstrate beginning leadership skills and participates in specific PCAFC experiences. Demonstrate creative approaches to the management of aspects of complex patient care. Provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering ,assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling. Possesses thorough knowledge of PCAFC, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure caregivers are provided with thorough, comprehensive care options within the scope of the CSP. Assess, plans, implements, and evaluates care based on age-specific components Coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care Influences care outcomes by collaborating with members of the interdisciplinary team Responsible and accountable for all elements of the nursing process when providing direct patientcare Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care Demonstrates knowledge and ability to apply principles of the CSP and CEAT practice Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
