Registered Nurse (RN) - Home Telehealth (HT) Care Coordinator

<p>THIS IS NOT A REMOTE POSITION The Registered Nurse (RN) - Home Telehealth (HT) Care Coordinator is responsible for providing competent, evidence-based care to assigned patients. The incumbent of this position works with potential non-VA community partners to improve access to rural and highly rural Veterans to healthcare services via telehealth initiatives and programming.</p><h3>Major Duties</h3><p>*THIS IS NOT A REMOTE POSITION. TELEWORK IS NOT AUTHORIZED.* The RN - HT Care Coordinator is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The incumbent assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Also, the HT Care Coordinator RN coordinates care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. The RN - HT Care Coordinator is also responsible for the following duties: Facilitating shared decision making with the Veteran, identifying Veteran needs/goals and development/modification of a patient-specific plan of care. Advocating for telehealth program needs at the facility, bringing matters requiring higher level intervention to the attention of facility leadership. Facilitating communication across primary care, specialty care, other healthcare professionals and team members, other clinics, and community agencies. Monitoring core quality and performance measures for all telehealth programs. Developing mechanisms to evaluate and monitor the quality, safety, efficiency, utilization, productivity, and effectiveness of telehealth. Other duties, as assigned.
