ABOUT THIS ROLE
<p>The Registered Nurse (RN) for Remote Patient Monitoring-Home Telehealth (RPM-HT) delivers knowledge-based, patient-centered care to Veterans across the care continuum. The RN provides comprehensive assessment, monitoring, and care coordination for Veterans in home and community settings using telehealth technologies.</p><h3>Major Duties</h3><p>Major Duties include, but are not limited to: - Provides comprehensive care coordination for Veterans enrolled in Remote Patient Monitoring-Home Telehealth (RPM-HT), including ongoing assessment, monitoring, and evaluation of clinical data such as vital signs, symptoms, and patient-reported responses. - Utilizes Home Telehealth technologies to identify changes in condition, triage health concerns, and facilitate timely interventions through the Patient Aligned Care Team (PACT), Emergency/Urgent Care, or community providers as appropriate. - Develops, implements, and evaluates individualized plans of care using the nursing process, integrating age-specific considerations, health teaching, counseling, and motivational coaching to support Veteran self-management of chronic and acute conditions. - Collaborates with interdisciplinary team members-including Home Based Primary Care (HBPC), Mental Health, Social Work, Pharmacy, Nutrition, and others-to review patient progress, share findings, and coordinate care transitions to ensure continuity and quality of care. - Provides health education and coaching to promote wellness, disease prevention, adherence to treatment plans, and improved health outcomes. Communicates regularly with Veterans between scheduled visits to ensure ongoing support and access to needed services. - Identifies gaps in care, recognizes patterns or trends in clinical data, and escalates findings that require provider assessment or intervention. - Participates in peer review, documentation improvement efforts, and continuous quality improvement initiatives within the HT program. - Performs clinical duties requiring physical stamina, including walking, standing, bending, lifting, and patient transfers with the use of mechanical lifts when needed. - Adheres to all required infection control and safety protocols, including wearing protective equipment when exposed to infectious patients, contaminated materials, or home environments requiring isolation precautions. - May encounter patients who are combative due to delirium, dementia, or psychiatric conditions and responds using appropriate safety techniques. - Will provide care in home or off-site locations. - Applies care management principles integrating health informatics, disease management, and telehealth technologies to ensure Veterans receive the right care, in the right place, at the right time. - Demonstrates beginning leadership skills appropriate by managing assigned workload, seeking guidance when needed, and contributing to program goals. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
