ABOUT THIS ROLE
<p>The Hampton VA Medical Center is a 1-C, teaching facility serving Eastern Virginia and Northeastern North Carolina. The main campus is located in Hampton, VA with 4 community-based outpatient clinics in Chesapeake, Portsmouth, and Virginia Beach, VA; and Elizabeth City, NC. Hampton VAMC provides comprehensive health care which includes primary care, acute inpatient, acute psychiatric, chronic spinal cord injury, a 169-bed domiciliary, and long-term Community Living Center/Hospice Care Unit.</p><h3>Major Duties</h3><p>Specific duties include, but are not limited to: Accountable for all elements of the nursing process (assessment, diagnosis, outcome identification, planning, implementation, evaluation) when providing and/or supervising direct patient care. Coordinates comprehensive care for Veterans, with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care. Streamlines referral processes by receiving, triaging, and directing timely, coordinated care utilizing a national standardized triage tool in accordance with VHA standards and guidelines. Applies clinical knowledge of internal referral care processes, review criteria, utilization management standards, and community care regulations to ensure appropriate care throughout the consult process. Serves as a liaison between internal and external providers, effectively guiding Veterans through a full range of care options within VA and the community. Supports timely access to care, empowering Veterans to make informed decisions and ensuring eligible Veterans are referred and scheduled appropriately. Triages incoming referrals, reviews available care options, provides scheduling guidance, and assists Veterans and providers throughout the consult process. Collaborates with the Office of Integrated Veteran Care to triage Requests for Services (RFS), promoting in-house access and optimizing utilization of VA resources. Contributes professional nursing perspective and clinical expertise to interdisciplinary teams, actively participating in quality improvement initiatives to enhance patient outcomes. Leads integration of evidence-based practice and research, assessing and evaluating patient outcomes for continuous improvement in quality of care and resource utilization. Utilizes delegation of authority, as appropriate, to function at the highest level of role performance; aligns the delivery of high-quality care with key performance indicators to ensure business practice standards consistently meet patient care needs.
