<p>The Registered Nurse (RN) in the role of a Quality Assurance& Performance Improvement (QAPI) RN applies the principles of the Holistic Approach to Transformational Change (HATCh) Model of Care. The incumbent functions directly to support the activities of the GEC Service and VA CLC's National Center for Enhancing Resources and Training (CONCERT) programs.</p><h3>Major Duties</h3><p>This RN will support CLC staff in creating high-functioning, relationship-based teams through specific quality improvement practices, thereby aiming to improve veteran experience and quality of care. This RN is the lead of the integrated quality care team established by the GEC service, and responsible for leading all CLC quality assurance and performance improvement (QAPI) measures per VISN/VACO guidelines. The RN is responsible for the planning, integrating, implementing, modifying, and evaluating the effectiveness of all quality initiatives within GEC service. RN will chair various CLC multi-disciplinary and process improvement meetings to review and improve quality measures, attend weekly IDT meeting, review nursing quality indicators and appropriateness of interventions. RN will Chair and facilitate monthly CLC QAPI meetings, prepare meeting minutes, and report to the hospital quality board or designated committee. RN is responsible for ensuring all GEC related Medical Center policies/SOPs are updated and maintained appropriately. Collaborate with Hines Quality & System Improvement service and CLC leaders to complete VISN action items, tracers, chart reviews, environment of care surveillance, and other duties directions per the service chief/designee. This RN will take lead in interpreting CLC Compare/SAIL data triggers and creating action plans to improve scores and Veteran care when applicable. RN will lead/participate in multiple facility wide committees representing GEC service. Initiate process improvement plan after identifying lack of compliance or opportunities for improvement. Participates in resident council meetings and collaborates with the CLC team to improve veteran satisfaction and cultural transformation. Serves as the CLC point of contact in creating action plans for survey findings, RCA, and audit/report action plan changes with the CLC Leadership Team and Hines Accreditation & Quality Service. Serves as a consultant to CLC staff during survey readiness, completions of pathways, and tracers. Serve as the key contact for CLC quality concerns and resolution of issues with VISN/VACO level. Functions as a consultant to CLC staff regarding CONCERT programs. Responsible to collaborate with Nurse Scientist/Researcher and serve as liaison to meet the GEC service requirements towards the Magnet or similar type of pathway to excellence. Responsible to review patient satisfaction findings, and work with GEC Leadership to complete service recovery and remediation action plans. The incumbent communicates both verbally and in writing with internal and external stakeholders resulting in evidence of robust Collaboratory Nursing System Redesign. The incumbent utilizes and implements LEAN Principles in change/project management at the team level and beyond. The incumbent understands and utilizes leading and lagging measures and other quality improvement data. Participates in strategic planning and alignment of goals with service level, facility level, and national organizational goals.
