ABOUT THIS ROLE
<p>Registered Nurse(RN)-Program Manager (PM) delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. The PM is a member of the Community Care leadership team, responsible for the delivery, coordination , utilization, quality, and cost effectiveness, and timeliness of health and health care services rendered to Veterans internal and external to the Detroit VA Medical Center.</p><h3>Major Duties</h3><p>Duties include: Executes position responsibilities that demonstrates leadership, experience, and creative approaches to the management of program aspects. Demonstrates performance and leadership that is broad enough to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care. Responsible for leading a facility wide program for all services within the organization that meets the requirements of National Directives. Oversee the provision of staff education, orientation, competencies and providing quality improvement consultation related to the program. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement & fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Participates in activities and strategies to sustain evidence-based-practice. Evaluates the program, collecting and submitting facility performance measures. Directs planning, policy and program development and implementation, quality assurance and process improvement activities for both Community Care and Referral Coordination programs. Responsible for analysis and reporting of community care and Referral coordination data, including but not limited to facility, VISN, and National KPIs and other metrics, community care usage, potential make/buy opportunities, and comparison of estimated vs. paid costs. Assures data integrity of community care dashboards and supporting data systems. Responsible for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Evaluates the program, collecting and submitting facility performance measures. Responsible for the documented outcomes at the program level and for supporting the program in all services within the organization. Program is defined as a facility wide system that meets the requirements of National Directives. Other duties as assigned Preferred Experience: 5 years of RN experience; 3 years of program management, data analysis experience.
