<p>At the VA Black Hills Health Care System, Primary Care nurses play a vital, hands-on role in supporting Veterans through compassionate, knowledge-based outpatient care. In this mission-driven setting, you will guide Veterans with education, wellness coaching, and coordinated care, helping them navigate their health journey while partnering closely with an interdisciplinary team to make a meaningful and lasting impact on those who served.</p><h3>Major Duties</h3><p>As a Primary Care Registered Nurse (RN) within the VA Black Hills Health Care System, you will deliver individualized, age-appropriate outpatient care while helping Veterans navigate their wellness journey. This role combines direct patient care, care coordination, health promotion, and collaboration with a supportive interdisciplinary team to positively impact the lives of those who served. Major Duties include, but are not limited to: Delivers direct patient care using the nursing process with a focus on assessment, planning, implementation, and evaluation. Coordinates care, provides patient education, and supports self-management across the continuum of care. Manages assigned patient panels, uses registries, and ensures timely evaluation and access. Provides health promotion, wellness coaching, and chronic care management. Collaborates with interdisciplinary team members and engages patients and families in care planning. Maintains therapeutic relationships and incorporates patient values and cultural needs into care. Assists with consult management, scheduling support, and follow-up communication. Prepares injections, supports clinical procedures, and adheres to aseptic and reusable medical equipment processes. Assists with specimen handling, dressings, postoperative care, and minor procedure support. Manages department supplies, devices, and compliance with safety, privacy, and computer security requirements. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
