Registered Nurse - Patient Aligned Care Team

<p>This Registered Nurse (RN) position is located at the Sarpy County Clinic in Papillion, NE. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. VA offers a comprehensive total rewards package: VA Nurse Total Rewards Duties of the Registered Nurse - Patient Aligned Care Team (PACT) include, but are not limited to: Provide evidence-based, gender-specific nursing care to Veterans in the Primary Care/PACT outpatient setting. Manage assigned patient panels, utilize clinical registries, and ensure timely access, evaluation, and coordinated care. Deliver health promotion, disease prevention, and chronic disease management education and coaching to patients. Support licensed vocational nurses and nursing assistants as appropriate to clinic operations. Participate in nursing education, staff orientation, competency development, and quality-improvement activities. Collaborate with the interdisciplinary team to improve holistic primary care services and promote optimal patient outcomes. Evaluate nursing practice using current evidence, provide constructive peer feedback, and contribute to an evidence-based practice culture. Support a safe, professional learning environment and share clinical knowledge and educational insights with colleagues.
