ABOUT THIS ROLE
<p>The VA Clinic in Lewiston Idaho is seeking an Intermittent, Patient Aligned Care Team (PACT) Registered Nurse (RN). The RN influences care outcomes by collaborating with members of the interdisciplinary team and will execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Intermittent employees work on as as-needed basis, with no regularly scheduled tour of duty or guaranteed hours.</p><h3>Major Duties</h3><p>Duties include but are not limited to: Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Provide gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provide health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Focus on individual growth and development in outpatient PACT nursing practice with increased levels of self-direction. Ability to formulate a plan of care based on nursing assessments. Seek guidance on how to improve care provided to patients and families. Delegate elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Bring primary care principles and theory with limited application of critical thinking skills. Foster a safe and supportive environment conductive to the professional development of health care professionals. Demonstrate performance and leadership that is broad enough to improve the care for the Primary Care Department and facility. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
