<p>The Patient Aligned Care Team (PACT) Registered Nurse (RN) is accountable for all aspects of the nursing process and is responsible for delivering and overseeing high-quality, direct patient care.</p><h3>Major Duties</h3><p>This is an 30 day announcement that will be used for the PACT RN Position at the Afton, WY Outpatient Clinic within the Sheridan VA Health Care System. Qualified applicants will be referred to the hiring manager beginning 6/24/2026 and thereafter until 7/9/2026. This Vacancy Announcement may be removed at any point during the open period. The PACT RN is responsible for coordinating comprehensive patient-centered care with a strong emphasis on education, self-management support, and overall patient satisfaction across the full continuum of care. Key responsibilities include, but are not limited to: Administering medications and performing clinical procedures in accordance with established policies and guidelines. Collaborating with interdisciplinary team members to influence care outcomes. This includes active participation in unit- or program-level quality improvement initiatives and customer service efforts. Delivering competent, evidence-based, gender-specific care to primary care patients, while providing appropriate oversight of licensed vocational nurses and nursing assistants. Providing direct patient care and care coordination for Veterans within the Primary Care Department. Managing assigned patient panels, utilizing registries, and ensuring timely access and appropriate evaluation for all patients. Promoting health and wellness through education, coaching, disease prevention, and chronic disease management strategies. Supporting nursing education, orientation, competency development, and contributing to quality improvement and outcomes initiatives. Demonstrating leadership in the delivery and enhancement of holistic primary care through collaborative approaches. Continuously evaluating clinical practice based on the best available evidence and offering constructive, informal feedback to peers. Fostering a safe, supportive, and growth-oriented environment for healthcare team members. Contributing nursing expertise during interdisciplinary team discussions and decision-making. Partnering with colleagues to drive positive change and optimize patient outcomes. Sharing knowledge to support safe, high-quality outpatient nursing care. Exchanging educational insights, experiences, and best practices with peers. Participating in strategies and initiatives that strengthen an evidence-based practice culture. Performing other duties as assigned. Permanent Change of Station (PCS): Not Available.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
