ABOUT THIS ROLE

<p>The VA New Mexico Healthcare System is seeking a Registered Nurse to serve on the Patient Aligned Care Team (PACT) - Outpatient Staff. They are responsible and accountable for all elements of the nursing process, including assessment, planning, implementation, and evaluation of age-specific care. Coordinates patient education and self-management, administers medications and procedures, collaborates with interdisciplinary teams, and demonstrates leadership in managing complex patient care.</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Provides comprehensive, gender specific primary care nursing services to Veterans within the Patient Aligned Care Team (PACT) model. Coordinates and manages care for assigned Veteran patient panels to ensure timely access, continuity, and quality outcomes. Utilizes patient registries, population health data, and clinical tools to identify and address care needs. Delivers health promotion, wellness coaching, disease prevention, and chronic disease management interventions. Serves as a clinical leader in the management of complex primary care patients and care coordination activities. Collaborates with interdisciplinary team members to develop, implement, and evaluate individualized care plans. Participates in the development, implementation, evaluation, and revision of policies, procedures, and clinical practice guidelines. Demonstrates leadership that positively impacts patient care outcomes, department performance, and organizational goals. Evaluates and implements evidence-based practices to improve care delivery for individuals, groups, and populations. Provides formal and informal mentoring, coaching, and constructive feedback to nursing peers and healthcare team members. Promotes a safe, supportive, and professional environment that fosters staff development and continuous learning. Monitors, analyzes, and reports program-level PACT outcomes to support quality improvement and performance initiatives. Leads complex quality, safety, and performance improvement efforts that enhance Veteran-centered care across the facility. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment medical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
