ABOUT THIS ROLE
<p>The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.</p><h3>Major Duties</h3><p>The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) will demonstrate the ability to cope with and manage competing priorities. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) independently plans and delivers technically competent care to patients with complex needs in alignment with the ANA Standards of Nursing Practice. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) manages the care of patients, while organizing and prioritizing patient care needs, and ensuring assigned tasks are completed. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) provides and directs others in Veteran/patient driven care that optimizes the patient's experience, satisfaction, and/or safety for a team of patients. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Role models and displays ethical behavior. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) demonstrates knowledge over a wide range of job responsibilities, effectively communicating with individuals and groups to build a strong sense of teamwork and purpose. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) actively contributes to the collaboration process and clearly communicates ideas to promote team success. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) makes meaningful and challenging contributions for the betterment of the work group.
