Registered Nurse - Patient Aligned Care Team (Outpatient)

ABOUT THIS ROLE
<p>The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Responsible to the assigned rating and approving supervisors.</p><h3>Major Duties</h3><p>Duties include but not limited to the following: Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department; Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management; Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels; Provides peers with informal constructive feedback for improvement; Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization; Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team; Integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations; Validates understanding of the rationale for care processes and decisions; Demonstrate the ability to cope with and manage competing priorities; Demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills; Collaborates with the interdisciplinary team to ensure the plan of care is achieved. Permanent Change of Station (PCS): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
