<p>The Fayetteville VA Medical Center is hiring a Registered Nurse for the PACT Outpatient team at our Sanford Location. The RN selected for this position will provide Veteran-centered care, coordinate services, and promote health education. Join our team and advance your nursing career today.</p><h3>Major Duties</h3><p>- Demonstrates advanced clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for an assigned panel of patients at Fayetteville Coastal Health Care System; recognizing the age-related cognitive, physical, emotional, and chronological maturation needs of the adult and geriatric patient focusing on chronic disease management. - Identifies and provides case management for high-risk cohort of patients utilizing chronic disease management data mining tools to include but not limited to Care Management Tool (CMT), Business Intelligent Strategic Learning (BISL), VISN 6 Access Report, VISTA, and VHA Support Service Center (VSSC) reports such as PC Almanac, Compass, Primary Care Management Module (PCMM) Care Needs Assessment (CNA). - Utilizes other registries to manage disease processes such as Diabetic, Mammography, Cardiovascular, and Opioid Registries to access data base for pain management. - The nurse is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care (ANA code of Ethics for Nurses). - Independently plans and delivers technically competent care to patients with complex needs. - The nurse, in all professional relationships, practices with compassion and respect for the inherent dignity, worth and uniqueness of every individual, unrestricted by considerations of social or economic status, personal attributes, or the nature of health problems (ANA code of Ethics for Nurses). - The nurse promotes, advocates for and strives to protect the health, safety and rights of the patient (ANA code of Ethics for Nurses). - Makes assignments which reflect the effective use of staff and prioritizes patient care needs. - Assesses patients to determine their needs and makes appropriate arrangements or referrals to provide for their needs. - Demonstrates compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment.
