<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>The Albany VA Healthcare System is seeking a Registered Nurse to work Full Time on the Outpatient Patient Aligned Care Team at the Westport VA Community Based Outpatient Clinic Location. The Patient Aligned Care Team (PACT) Staff Registered Nurse is responsible for providing competent, evidence-based care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting, and provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. The Registered Nurse is responsible for all elements of the nursing process when providing and/or supervising direct patient care. The incumbent will assume responsibility for the coordination of care focused on patient education, self-management, customer satisfaction throughout the continuum of care, and administer medications and procedures per established policies and guidelines. He/she Influences care outcomes by collaborating with members of the interdisciplinary team, and will actively participate in the unit/program level quality improvement process and initiatives as well as customer service programs. The duties include, but are not limited to: Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provide health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Contribute professional outpatient nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Demonstrate leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluate practice in an ongoing process, based on best evidence. Provide peers with informal constructive feedback for improvement. Question PACT clinical practices for the purpose of providing evidence-based care. Participate in activities and strategies to sustain an evidence-based-practice culture. All other duties as assigned. Work Schedule: Full-time.
