Registered Nurse (Outpatient Staff)

<p>This Registered Nurse- Outpatient Staff is located at the Kansas City VA Medical Center in Kansas City, MO within Medical Sub Specialty department. Responsible for providing competent, evidence-based specialty care to assigned patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seeks guidance on how to improve care provided to patients and families. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Demonstrates leadership in delivering and improving holistic, specialty care through collaborative strategies with others. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for specialty care outpatients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality specialty nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
