ABOUT THIS ROLE
<p>The Registered Nurse - Outpatient Patient Aligned Care Team demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>The Patient Aligned Care Team (PACT) Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific primary care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Other duties included but not limited to the following: Manages nursing care of patients, organizes, and completes own assignments in a safe and timely manner, assisting team members as necessary. Administers medications and procedures per established policies and guidelines. Integrates patient preferences into daily delivery of quality patient care. Demonstrates the ability to communicate confidently with members of the healthcare team and demonstrate professional behaviors that promotes cooperation and teamwork. Influences care outcomes by collaborating with members of the interdisciplinary team. Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care. Fundamental primary care experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Treats all patients, families and other caregivers with dignity and respect incorporating knowledge, values, and beliefs into nursing care delivery without judgement or discrimination. Provides the Veteran's family, caregiver and/or significant other, the Veteran's representative, visitors to VA facilities, all VA staff, and other customers with consistent information according to establish policies and procedures.
