<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>GENERAL DESCRIPTION: This nursing position falls under the Nursing, Mental Health Service which serves and coordinates medical and mental health needs in various programs. Duties include but are not limited to: Implements approved policies and procedures; completes required documentation in accordance with VHA, JCAHO, SOARS regulatory and accreditation requirements Assists with ensuring completion of the relevant comprehensive assessments, documentation and implementation of interdisciplinary treatment plan for all patients Functions as an integral member of the treatment team in support of the Behavioral Health Interdisciplinary Program (BHIP) Coordinates and assist the physician/nurse practitioner with behavioral and medical aspects of patient's care Supports the mission and vision of the agency/facility/service Provides education and counseling on disease prevention, health promotion, and medication management; ensures safe handling and ongoing compliance of patients with medication administration Participates in performance improvement initiatives; ensures completion of relevant clinical reminders Makes notification and referrals to appropriate discipline(s) based on assessed needs Completion of required documentation in accordance with VHA, JCAHO, CARF regulatory and serves as team leader and case manager for an assigned panel of patients Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Independently provides patient-centered and recovery-oriented care and maintains self-awareness while caring for patients living with MH conditions. Actively participates in care coordination, liaising with Inpatient MH Units to facilitate continuity of care, MH interdisciplinary treatment team huddles and meetings and responds to psychiatric and medical emergencies as directed. Reports and addresses safety concerns, document nursing triage assessments, risk screens, clinical reminders, and patient plan of care, according to facility policy. Maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Other duties as assigned.
