ABOUT THIS ROLE
<p>The Registered Nurse (Nurse Physician Liaison) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. They assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They administer medications and procedures per established policies and guidelines.</p><h3>Major Duties</h3><p>https://youtu.be/LM35k1p_3Tc?si=OYdVLJgrv6Jbwh7l Duties and responsibilities include, but are not limited to, the following: Act as a liaison between physicians and nursing/ancillary staff. Document tentative plan of care and disposition for patients. Document discharge planning for in-patients on assigned medicine teams. Document Home Nursing/Hospice plans and referrals. Plan for discharge/transfer of patients and completes discharge note in CPRS. Disseminate a list of daily discharges with disposition plans to multidisciplinary discharge team. Oversee the Discharge Expediter and communicates updates throughout day as to patient discharge needs. Serve as a nurse/physician advocate. Serve as an educator to inpatient physicians regarding CPRS and hospital policies and procedures. Provide individualized patient care consistent with identified knowledge of patient's strengths and limitations. Is proactive in identifying potential crisis situations and responds appropriately. Initiate referrals and/or consultations as appropriate to achieve the multidisciplinary plan of care. Function as a consult person to physicians, nursing and other personal for complex patient care issues. Function as an inpatient case manager, insures that post hospitalization follow-up and discharge needs are completed prior to patient discharge. Arrange for procedures, scheduling of diagnostic tests, and transportation home. Complete consults and forms for home oxygen and O2 flight orders. Work closely with assigned physicians to determine best plan of care for patient. Order supplies for assigned patients (i.e. Wound care, PICC line, home infusion, and enteral feedings). Collaborate with assigned Pharm D for Coumadin/Enoxaparin therapy follow-up. Work closely with IT to coordinate the computer needs for the Medicine Service. Assist with training and administrative orientation for house staff and faculty.
