The VA Northeast Ohio Healthcare System is recruiting for three full-time Registered Nurse Intensive Community Mental Health Recovery (MHICM) Case Managers. These positions, located within Patient Care Nursing Service under Psychiatry/Mental Health Nursing, provide patient-centered, recovery-oriented care to individuals living with mental health conditions.</p><h3>Major Duties</h3><p>The 2-page Resume requirement does not apply to this occupational series. The Registered Nurse-MHICM Case Manager duties include but are not limited: Provides clinical community-based case management services to Veterans with severe mental illness (SMI), severe functional impairment, and high Inpatient MH unit utilization, in coordination with existing community and Veterans Affairs (VA) services. Gains awareness of the principles and goals of outpatient MH treatment by understanding practices related to patient-centered care, recovery-oriented principles, telemental health, community-based care, and self-awareness related to caring for patients living with MH conditions. Attends and participates in MH interdisciplinary team huddles and meetings, uses therapeutic communication techniques, practices de-escalation techniques, utilizes recovery-oriented language, and provides Veterans, their family members, and/or significant others with education regarding their illnesses, medications, and treatment plan. Clinically provides outpatient, community-based MH services for a subgroup of Veterans with SMI; including but not limited to providing transportation, in-home and community based visits, and responding to psychiatric and medical emergencies as appropriate. Responsibilities include utilizing interventions which maximize health capabilities, including health promotion, pain management education, monitoring vital signs, chronic disease assessments and education, whole-health interventions (e.g., aromatherapy), facilitating virtual access to specialty care (e.g., Wound Care Nurse, TeleHealth), and care coordination are incorporated to support recovery and independence and reduce unnecessary urgent care and emergency room visits. Maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient.
