<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Portland VA Medical Center consists of the main tertiary care medical center overlooking the city of Portland, the Vancouver Campus in Vancouver, Wash., and ten outpatient clinics across central and northwest Oregon. Our health care system provides a full continuum of inpatient, outpatient, long-term, and emergent care and is proud to host several national-level Centers of Excellence leading research efforts from mental illness to Parkinson's, Epilepsy to auditory research, and others. The Mental Health- New Patient Access Team Registered Nurse (MH-NPAT RN) practices in the Outpatient Mental Health Clinic. Duties include, but not limited to: Responsible for screening new to MH patient referrals, facilitating follow up care for established patients, collaborating with treatment providers and managing MHD consults and self-referrals such as VAOS, phone calls and voice mails to the NPAT line. Case management and consultation may take place individually, in groups, by telephone or other virtual methods as appropriate. Serves as a mental health resource consultant and educator for all VA staff and outside partners offering a global view of organizational processes and program resources. Uses professional standards of care and collaborates with other services outside and within the VA to ensure safe, efficient MH care for veterans. Shares expertise with VA staff to resolve MH access and care challenges and improve organizational performance. Possesses experience in mental health nursing as well as knowledge of VA and community resources. Responsible for the documented outcomes at the program or service level.
