<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>The Mental Health (MH) Clinical Nurse Navigator (CNN) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others for all areas included within assignments. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers. Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture. Identifies care and cultural disparities between assigned units and reports disparities within Shared Governance Structure and Management. The primary purpose of the MH CNN is to improve the safety of patient care and to better prepare nurses to provide mental health services in an environment outside of the mental health care environment. The MH CNN role engages as a highly skilled clinician in outcome-based safe practices. The MH CNN provides daily collaboration through rounding with inpatient and outpatient teams in the standards of delivery of care and education for patient and family regarding mental health disease process/status and provides services per the Nursing Plan of Care The MH CNN provide active collaboration with staff nurses for a distinct group of patients by providing MH assessment, identifying the nursing care plan gaps and work with staff in adjusting the gaps and if necessary, actively provides direct patient care in complex MH situations. The primary purpose of the Mental Health Clinical Nurse Navigator (MH CNN) is to improve the safety of patient care and to better prepare nurses to provide mental health services in an environment outside of the mental health care environment as well as assess discharged patients for continued safety via TeleHealth modalities. Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
