ABOUT THIS ROLE
<p>The Registered Nurse, Mental Health Clinical Nurse Coordinator, assists with the significant collaboration between the community and Veterans Integrated Systems Network (VISN) healthcare personnel in the transition of Veterans through the Veterans Affairs (VA) healthcare system.</p><h3>Major Duties</h3><p>The Registered Nurse, Mental Health Clinical Nurse Coordinator, performs duties that include but are not limited to the following: Responsible for the triage and care coordination of complex patients, works closely with Veterans to ensure smooth transition of care to community or VA facilities and care related to referrals, improvement meetings, and service meetings. Demonstrate performance and beginning leadership skills that impact the standard of care for all patients throughout the continuum of care. Responsible for completing services within the organization that meets the requirement of National Directives. Provides quality and performance measures analysis and reporting of consult trends and system issues through the utilization of the nursing process. Collaborates with the interdisciplinary team to improve the delivery of patient care. Provides exceptional communication to all parties involved in the delivery of care, to include the Veteran, family, medical staff, community partners, etc. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice change for individuals, groups, and populations. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
