<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>At St. Cloud VA Health Care System, Registered Nurses (RN) in Mental Health Acute Psychiatric care plan, integrate, and deliver high-quality, timely care for Veterans-educating, counseling, and collaborating with teams to support safe transitions and stronger outcomes across the continuum of care. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care. Serving as a liaison to internal and external providers and managing the care of the Veteran and the seamless transition to and from the community. Following the standardized care coordination model, incorporating case management principles, and processes related to referral, authorization, and secure medical documentation exchange. Identifying Veterans requiring disease management and obtaining resources internal or external to the U.S. Department of Veterans Affairs (VA), as applicable. Monitoring, tracking, and intervening, if necessary, in the episode of care across the continuum. Collaborating and co-managing the Veteran's care with the multidisciplinary team in a cost-effective manner. Providing care and counseling, health teaching, health counseling, case finding, and executing medical regimens including administering medications and treatments prescribed by authorized providers. Actively participating in unit/program level quality improvement processes, initiatives, and customer service programs. Demonstrating understanding and use of the Electronic Health Record and Bar-Coded Medication Administration (BCMA) software per policy. Participating in nursing service committees and in High Reliability Organization (HRO) processes and HRO initiatives.
