ABOUT THIS ROLE
<p>Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.</p><h3>Major Duties</h3><p>* Influences care outcomes by collaborating with members of the interdisciplinary team. * The Lung Cancer Screening Clinical Coordinator Registered Nurse (RN) focus is on individual growth and development in practice with increased levels of self-direction. * This nurse brings principles and theory with limited application of critical thinking skills. * Provides patient management, care coordination, and disposition planning for patients. * Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. * Coordinates care with multiple providers across all levels and sites of care. * Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients. * The Lung Cancer Screening Clinical Coordinator manages consults for potential lung cancer screening enrollment by performing chart reviews to verify eligibility and appropriateness. * Conducts shared decision-making interactions with potential lung cancer screening enrollees. * Organizes and leads in-services to educate and share information about the Lung Cancer Screening Program with clinical staff. Facilitates follow-up referrals to evidence-based smoking cessation care. * Optimizes patient-centered care practices through patient education, care management, screening exams, procedures, and clinical appointments. * Maintains accountability for following imaging studies and their recommendations. * Collects and analyzes program data from a variety of sources. * Provides regular communication and reports as needed to the interdisciplinary team. * Performs initial and ongoing monitoring and tracking of abnormal images/lung nodules and communicating with providers to ensure that they are alerted to abnormal findings when necessary. *The Lung Cancer Screening Clinical Coordinator addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. * Coordinates and expedites care from diagnosis to treatment. * Evaluates care and outcomes to ensure timely and appropriate provision of services. * Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs. * Collects, analyzes, and reports program evaluation data for assessment on the impact of implementing lung cancer screening and to make evidence-based improvements.
