ABOUT THIS ROLE
<p>The Interfacility Care Coordinator Registered Nurse (RN) in Quality and Patient Safety is responsible for providing leadership in delivering competent, evidence-based care to assigned patients, integrating knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. This nurse demonstrates competence in practice, decision-making, deliberate planning, and critical thinking skills.</p><h3>Major Duties</h3><p>Major Duties of the Registered Nurse - Interfacility Care Coordinator (To include but not limited to): Monitors program delivery of critical medical, technical, and program support necessary for patient management, care coordination, and interfacility collaboration for remote and outpatient settings. Consistently conducts and reviews coordination of telehealth and face-to-face care between VA locations and ensures that Veterans, family members, and caregivers receive the most appropriate options and services to meet their complex health care needs. Provides leadership in collaborating between ordering or referring VA providers and specialty care services. Ensures that systems are in place and used to triage consults and referrals and confirm the appropriate care is requested and all necessary pre work and tests are completed. Leads in the program implementation and coordinating transition of care between services or locations, to minimize fragmentation of healthcare delivery, and optimize quality of patient and family outcomes in a cost-effective manner. Utilizes telephone triage, creates professional assessment and Page 1 Functional Statement: Nurse I Level 3 plan, expedites delivery of multiple health care services, and advocates for holistic care through patient patient/family education and provider interaction to meet patient care needs. Validates understanding of the rationale for care processes and decisions, questions clinical practices for the purpose of providing evidence-based care and demonstrates the ability to cope with and manage competing priorities.
