Registered Nurse (Inpatient Case Management)

ABOUT THIS ROLE
<p>VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking to hire an experienced Registered Nurse to work in our Inpatient Case Management Service. Incumbent is expected to function independently, collaboratively, showing leadership and communicating effectively, coordinating care for complex/high risk patients, for a seamless, efficient and safe discharge.</p><h3>Major Duties</h3><p>Current, permanent GLA employees will be given the first consideration. Must submit most recent SF-50 to receive 1st consideration. Job duties include, but are not limited to, the following: Conducts comprehensive patient assessments. Conducts comprehensive risk assessments to identify high-risk Veterans and collaborates with the care team to create personalized care plans that mitigate harm. Coordinates complex care. Facilitates communication and coordination of care among members of the interprofessional health care team. Leads and attends interdisciplinary meetings/rounds. Applies specialized skills, knowledge, and competencies throughout the case management process. Identifies barriers to care and client's engagement in own health. Initiates discharge planning upon admission. Applies InterQual Criteria and/or DRG and discusses with the interdisciplinary team. Initiates patient care conferences. Educates the patient, the family or family caregiver, and members of the interprofessional health care team. Coordinates and ensures appropriate resources are in place prior to discharge. Implements care interventions. Provides referrals to various service resources along a continuum of care. Monitors and evaluates the individualized plan of care. Facilitates the primary care provider assignment for patients with no primary care provider. Ensures post-operative and follow-up care is arranged and documented prior to discharge. Engages in performance improvement activities. Utilizes computer programs effectively, i.e. Microsoft Word, PowerPoint, Excel. Adheres to the Standard Operating Procedure for Inpatient Case Management and relevant policies, procedures, directives.
