<p>The Registered Nurse (RN) Home Telehealth Care Coordinator executes position responsibilities that demonstrates leadership, experience, and creative approaches to the management of complex patient care. Care will be provided in the least restrictive, most cost-effective manner. THIS IS NOT A REMOTE JOB POSITION- this position works in office in the SLC VAHCS George E. Whalen Medical Center Monday to Friday.</p><h3>Major Duties</h3><p>https://youtu.be/LM35k1p_3Tc?si=6wdZw0Lag331XBMR Duties and responsibilities include, but are not limited to, the following: Deliver fundamental knowledge-based care to assigned clients while developing technical skills. Assess, plan, implement and evaluate care based on age-specific components. Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administer medications and procedures per established policies and guidelines. Influence care outcomes by collaborating with members of the interdisciplinary team. Identify appropriate Veterans for enrollment and discharges patients to transition from the program. Collaborate and engage in processes that assess, advocate, plan, implement, coordinate, monitor, and evaluate healthcare options and services so that they meet the needs of the individual Veteran. Per the Remote Patient Monitoring- Home Telehealth Manual Section 6, RN Care Coordinators are case managers and health coaches who are highly skilled and have specific training and competency in the use of disease management, health care informatics, and remote patient monitoring technologies. Case management is the cornerstone of Remote Patient Monitoring -Home Telehealth. Thorough, regular chart reviews and daily monitoring of data are only part of the overall management of care. There is a primary focus on enhancing patient self-management, education on the disease-specific elements of care, and oversight of health-related behaviors that are associated with positive patient outcomes. Monday - Friday Telework: This position is authorized for telework-situational (ad-hoc) AND has NOT been authorized as remote position. (NOTE: The Veterans Health Administration, including VISN 19 and the VA SLC Health Care System, will comply fully with the intention and instructions contained in the "Return to Office" Executive Order. This position will likely be eligible to telework on a limited Ad-Hoc basis only). Virtual: This is not a virtual position. Permanent Change of Station (PCS): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
