ABOUT THIS ROLE

<p>As a Home Health Registered Nurse at the Sioux Falls Veterans Affairs Health Care System, you will bring high-quality, Veteran-centered care directly into the home and community setting. This meaningful role allows you to build trusted relationships with Veterans and their families while using your clinical judgment, care coordination, advocacy, and collaboration skills to help Veterans remain safe, supported, and connected to the care they need.</p><h3>Major Duties</h3><p>The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Provide competent, evidence-based patient care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Demonstrate leadership in delivering and improving holistic care through collaborative strategies with others and deliver care in accordance with established standards, policies, and procedures consistent with the interdisciplinary plan of care. Provide care to the Veteran in their home setting utilizing proper precaution. Assess and chart observations of the Veteran's condition at each visit. Complete evaluation tasks that include medication review, vital signs, wound care, and other duties within the Registered Nurse scope of practice. Coordinate plan of care with providers and other interdisciplinary team members. Focus on the Veteran in the context of family/community by integrating an assessment of living conditions, individual and family dynamics, and cultural background into the Veteran's plan of care. Perform supervisory visits on the cares provided by Licensed Practical Nurses and Nursing Assistants within a timely manner. Serve as an advocate in the care management process by supporting decisions, clarifying options, and problem solving with the interdisciplinary team; provide consultation and guidance and act as a liaison for staff, nursing homes, and other health care providers regarding Veteran needs in the community/home setting and in the interpretation of Department of Veterans Affairs rules and regulations. Driving Requirement: - You may be expected to travel 76% or greater of the time for this position to various care locations within the service area, including Veteran homes as part of Home Health care delivery. - You must possess and maintain a current, valid, unrestricted state-issued driver's license. - You must provide a safe, clean driving record from the Department of Motor Vehicles (DMV) during onboarding. - You must successfully complete a driver's physical examination to demonstrate medical ability to safely operate a government-owned or leased vehicle, as required under 5 CFR 930 and HR Job Aid 05-OB-010.002. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Selected applicants will be required to complete an online onboarding process. Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents). Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
