<p>The Home & Community Based Care, Community Adult Day Health Care program partners with community adult-day health care sites to support Veterans in need of assistance with activities of daily living, isolated, would most likely be in a nursing home but funding/resources are not currently available, or when their caregiver is experiencing burden. The Registered Nurse plays a critical role in linking Veterans to appropriate community facilities and providing complex case management.</p><h3>Major Duties</h3><p>The Home and Community-Based Care (H&CBC) Registered Nurse assigned to the Community Adult Day Health Care (CADHC) program is responsible for providing complex clinical care coordination and case management for medically and psychosocially complex Veterans. This role ensures timely access to services, regulatory compliance, interdisciplinary collaboration, and continuity of care across multiple community-based settings while supporting Veterans, caregivers, and community partners throughout the care continuum. Duties of the CADHC RN include, but are not limited to: Processing all consults in accordance with strict timelines Assessing and determining medical appropriateness for the CADHC program Completing onsite admission for Veterans accepted into the program Performing quarterly (and as needed) onsite visits to assigned CADHC Veterans. Providing ongoing case management and care coordination to assigned CADHC Veterans Performing facility inspections and investigations Participating in weekly interdisciplinary team meetings Participating in community outreach activities. PCS Appraised Value Offer (AVO): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Complete all application requirements detailed in the "Required Documents" section of this announcement.<h3>Education</h3>The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE). If your school was accredited by ACEN or CCNE at the time of graduation, but is no longer listed on the websites, you must provide documentation of accreditation with your application packet. If you are using foreign education to meet qualification requirements, you must send a Certificate of Foreign Equivalency with your transcript in order to receive credit for that education.
