<p>The Marion VA Health Care System is seeking a Registered Nurse (RN) to serve in the role of RN Care Manager for the Home Based Primary Care team with Extended Care, Nursing Service located in Marion, IL. The Registered Nurse in this position provides health care services and care management to high risk patients in their homes.</p><h3>Major Duties</h3><p>The Care Manager is accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking an individual who possess good organizational skills which enable completion of work, demonstrated ability to assume leadership in a patient centered program and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. He/she assesses, plans, implements and evaluates patient care needs assuring quality of care. The incumbent provides professional nursing care that includes, but is not limited to: Case Management. Patient Assessments. Participation in interdisciplinary team process. Home care visits. Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care). Completes and submits clinical notes. Observes patient for changes in conditions and reports appropriately. Provides education instructions to patients and their designated significant other. This position requires that the employee operate a government motor vehicle. Performs other professional nursing duties as assigned and within their scope of practice. Other duties as assigned. PLEASE NOTE: These positions require Basic Life Support (BLS) certification. If selected, Applicants will provide proof of current certifications prior to employment. Only BLS certification which included Automated External Defibrillator (AED) obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
