Registered Nurse - Home Based Primary Care

<p>The Marion VA Health Care System is seeking a Registered Nurse (RN) to serve in the role of RN Care Manager for the Home Based Primary Care team with Extended Care, Nursing Service located in Marion, IL. The Registered Nurse in this position provides health care services and care management to high risk patients in their homes.</p><h3>Major Duties</h3><p>The Care Manager is accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking an individual who possess good organizational skills which enable completion of work, demonstrated ability to assume leadership in a patient centered program and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. He/she assesses, plans, implements and evaluates patient care needs assuring quality of care. The incumbent provides professional nursing care that includes, but is not limited to: Case Management. Patient Assessments. Participation in interdisciplinary team process. Home care visits. Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care). Completes and submits clinical notes. Observes patient for changes in conditions and reports appropriately. Provides education instructions to patients and their designated significant other. This position requires that the employee operate a government motor vehicle. Performs other professional nursing duties as assigned and within their scope of practice. Other duties as assigned. PLEASE NOTE: These positions require Basic Life Support (BLS) certification. If selected, Applicants will provide proof of current certifications prior to employment. Only BLS certification which included Automated External Defibrillator (AED) obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted.
