<p>The Home Based Primary Care Program Director Registered Nurse position is essential for overseeing the Home Based Primary Care (HBPC) program and requires specialized knowledge of HBPC mission, culture, and operational processes to provide safe, patient-centered care for Veterans with geriatric syndromes and palliative/hospice care needs.</p><h3>Major Duties</h3><p>The Home Based Primary Care Program Director requires a highly skilled RN who has excellent home based primary care experience encompassing clinical and administrative skills. Major duties include, but are not limited to: - Provides executive-level oversight of five (5) Home Based Primary Care (HBPC) teams, ensuring effective coordination of care, optimal staffing, and consistent application of evidence-based practice. Serves as operational program director, demonstrating advanced judgment and accountability at the program level. - Serves as the lead for Joint Commission (JC) and other regulatory surveys across the Washington DC VA Medical Center, leading system-wide readiness activities, engaging interdisciplinary partners, and implementing strategies to correct negative trends beyond the immediate practice area. - Provides clinical and administrative oversight of HBPC interdisciplinary teams, using advanced programmatic knowledge to guide practice standards, evaluate outcomes, and ensure high-quality, evidence-based care for complex HBPC patients. - Conducts comprehensive, expert-level assessments of multifaceted medical, behavioral, and psychosocial needs in the home environment, demonstrating advanced clinical judgment and influencing care decisions at the program level. - Develops, evaluates, and adapts individualized care plans, integrating evidence-based guidelines and contributing to program-level improvements in care planning processes. - Implements advanced clinical interventions aligned with patient diagnoses, goals, and evolving health conditions, leveraging broad expertise to ensure safe, effective, and resource-efficient care. - Leads patient, family, and caregiver education strategies, promoting health literacy and population-level improvements in disease self-management through evidence-based teaching methods. - Collaborates and communicates at the expert level with physicians, therapists, social workers, and other interdisciplinary partners, initiating and leading efforts to improve care coordination and Veteran outcomes beyond the immediate team. - Ensures accuracy, timeliness, and comprehensive documentation in the electronic health record (EHR) and promotes system optimization and data integrity that contribute to improved program performance. (Supported under Informatics RN/Nurse III expectations related to documentation quality.) - Ensures HBPC program compliance with regulatory standards-including JC, OIG, CMS-and leads initiatives to maintain continuous survey readiness across the care continuum. - Upholds and enforces HIPAA and privacy safeguards, modeling high-reliability and ethical care principles across all HBPC teams. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical evaluation. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
