ABOUT THIS ROLE
<p>The Home Based Primary Care Registered Nurse provides direct Veteran care and care coordination within the HBPC Program. The nurse is responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. This nurse plans and delivers technically competent care in the homes of Veterans with complex needs, including health promotion, disease prevention, geriatric care, primary care, and palliative care.</p><h3>Major Duties</h3><p>Duties of the position: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to managing complex Home based Primary Care (HBPC). Provides direct Veteran care and care coordination within the HBPC Program. Performs case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Plans and delivers technically competent care in the homes of Veterans with complex needs, including health promotion, disease prevention, geriatric care, primary care, and palliative care. Utilizes interventions that maximize health capabilities and reduce unnecessary urgent care and program utilization. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines. Demonstrates performance and leadership broad enough to improve care for the HBPC Program and the facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through HBPC knowledge sharing to promote safe, high-quality nursing care, and foster a supportive environment conducive to professional development. Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations. Maintains responsibility for documented HBPC outcomes at the program or service level. Demonstrates broad, complex HBPC outcomes at applicable organizational levels within the facility.
