<p>The Registered Nurse will provide care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems.</p><h3>Major Duties</h3><p>https://www.youtube.com/watch?v=NpaTtyybXFQ Duties and responsibilities include, but are not limited to, the following: Execute position responsibilities that demonstrate beginning leadership skills and participate in specific program experiences. Demonstrate creative approaches to the management of HPDP program aspects of complex patient care. Demonstrate performance and beginning leadership skills that start to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care. Advice facility and primary care leadership on clinical and administrative issues related to HPDP, including clinical preventative services. Advocate, lead or participate in policy, program development, and resource allocation to support HPDP programs and services for Veterans. Lead the effort through data collection and analysis to identify HPDP challenges, priorities for improvement, and opportunities for disseminating and sustaining best practices. Primary point of contact for national health promotion campaigns and events, ensuring local implementation and participation. Coordinate the development of user-friendly tool to assist in successful implementation of HPDP programs and services. Collaborate with community partnerships and resources, and Whole Health. Provide leadership in the development or modification of service delivery processes to support the successful implementation of HPDP services. Responsible for leading a facility wide program for all services within the organization that meets the requirements of National Directives. Oversee the provision of staff education, orientation, competencies, and provide quality improvement consultation related to the program specifically. Ensure compliance with VHA Directive 1120.02 and VHA Directive 1120.05 by implementing nationally required components of the Health Promotion and Disease Prevention Program, including coordination and performance monitoring of the Healthy Living Assessment (HLA) and the integration of clinical prevention services across the continuum of care. Lead facility-wide coordination of Health Promotion and Disease Prevention activities by chairing or co-chairing the facility HPDP Committee and supporting the Healthy Living Team to integrate HPDP principles into service line operations. Serve as the primary facility liaison to VISN HPDP leadership and the National Center for Health Promotion and Disease Prevention (NCP), ensuring accurate reporting, effective communication, and successful implementation and national and VISN initiatives aligned with Montana VA Health Care System strategic priorities.
