<p>The Dayton VA Medical Center is currently recruiting for a RN, Patient Care Coordinator Nurse Expediter within the Patient Business Service Community Care unit.</p><h3>Major Duties</h3><p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards Major duties include but not limited to: Demonstrates a high level of clinical practice, knowledge, and leadership skills to improve patient access, flow, and positively impact the facility utilization of services. As a clinical reviewer he/she serves as the clinical liaison with medical center staff, and internal customers. He/she collects, interprets, and communicates pertinent clinical data, and applies lnterQual criteria. Serves as the primary point of contact for all inpatient transfers into, and out of, the Dayton VAMC from other healthcare organizations; Dayton VAMC long-term care facility admissions; is the Inter-Facility Consult Coordinator for the Dayton VAMC. As the clinical bed control RN coordinator, he/she will collaborate with appropriate disciplines and identify bed availability in Bed Management Software (BMS) to manage acute care patient flow at this healthcare system. Accurately documents care of clients. Conducts clinical reviews to interpret and evaluate complex data directly related to diagnosis, condition, and treatment to assure appropriate medical necessity, appropriate care coordination and timely utilization of non-VA health care services. Provides care in a safe and cost-effective manner. Observes safety practices, maintains safe work environment, and follows procedures for reporting unsafe conditions and ensuring corrective action to prevent potential hazardous situations. Performs other duties as assigned.
