<p>This Endoscopy Care Coordinator Registered Nurse within Gastrointestinal Specialty at Kansas City VA Medical Center, demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed for managing complex client situations. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. Demonstrates the ability to cope with and manage competing priorities. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients. Makes sustained contributions to healthcare by actively participating in healthcare facility or service-level committees or councils, establishing active relationships and demonstrating leadership in improving care beyond the immediate practice setting. Leads with emotional and social intelligence. Demonstrates resilience in coping with challenging and stressful situations, to assist others in increasing their self-determination and competence beyond the immediate practice setting Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. You may be required to serve a probationary period.
