<p>The Western New York VA Medical Center is seeking a full-time Registered Nurse to work on the Dialysis Team at the Buffalo VA Location. The Dialysis Registered Nurse is a professional nurse with specialized knowledge and skills who demonstrates leadership when providing, directing, and improving care for patients with end-stage renal disease, acute kidney injury, and late-stage chronic kidney disease.</p><h3>Major Duties</h3><p>The Dialysis Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. He/she will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The incumbent will administer medications and procedures per established policies and guidelines; while influencing care outcomes by collaborating with members of the interdisciplinary team. (The core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.) The incumbent will function as a primary nurse for end stage renal disease patient panel and serves as key resource assisting patients navigate the healthcare system, and complete documentation for all clinical and technical recordkeeping in the electronic health record and treatment flowsheets in accordance with established policies and procedures. The Duties and Responsibilities include but are not limited to: Complete care of dialysis patients including initiation, intradialytic monitoring , and discontinuation of treatment Administer medications and procedure per established hospital and unit policies Maintains all Hemodialysis Unit-specific competencies Collect health data, make a nursing assessment(diagnosis) before the initiation of dialysis, and document any changes during treatment Assesses the need for isolation of a patient and/or dialysis machine based on laboratory data according to department protocol Implement the nursing plan of care by providing direct nursing care for the patient in acute and chronic renal failure Evaluate the plan of care by observation, reporting and recording Active participation in quality improvement measures Use a team approach to identify, analyze, and resolve patient care problems with measurable outcomes Question clinical practices for the purpose of providing evidence-based care All other duties as assigned Work Schedule: Full-Time. Variable Day (10 or 12 Hour Tours) - (7:30 am - 6:00 pm and 9:30 am - 8:00 pm are shifts) with mandatory on-call duties.
