<p>Join VA Black Hills Health Care System Hot Springs as a Dialysis Registered Nurse and deliver meaningful, Veteran-centered care to patients with kidney disease. In this role, you will provide safe hemodialysis treatment, coordinate care, educate patients, support self-management, and collaborate with an interdisciplinary team to improve outcomes across the continuum of care.</p><h3>Major Duties</h3><p>At VA Black Hills Health Care System Hot Springs, the Dialysis Registered Nurse helps Veterans with end-stage renal disease, acute kidney injury, and late-stage chronic kidney disease manage complex care with confidence, dignity, and continuity. This role offers the opportunity to build therapeutic relationships with patients and families, provide safe dialysis treatment, support education and self-management, and collaborate with an interdisciplinary team to improve outcomes, strengthen care coordination, and enhance the Veteran experience Duties include, but are not limited to: - Deliver knowledge-based nursing care for Veterans receiving dialysis services, including assessment, planning, implementation, and evaluation of individualized care. - Provide and support safe hemodialysis treatment, including treatment start-up, machine programming, monitoring during treatment, and treatment completion. - Serve as a primary nurse for an assigned end-stage renal disease patient panel, helping Veterans navigate care needs across the healthcare system. - Coordinate care focused on patient education, self-management, continuity, and patient satisfaction throughout the care continuum. - Administer medications and perform procedures in accordance with established policies, guidelines, and dialysis care standards. - Collaborate with interdisciplinary team members and other departments to achieve desired patient outcomes and support shared care delivery decisions. - Monitor, track, trend, and report care continuity needs and patient outcomes to the interdisciplinary team. - Apply specialized dialysis knowledge, including vascular access anatomy, dialysis catheter types, treatment pathophysiology, infection control, and disinfection practices.
