<p>The Community Nursing Home Liaison Registered Nurse executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management as well as provide patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.</p><h3>Major Duties</h3><p>https://www.youtube.com/watch?v=NpaTtyybXFQ Major Duties: Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress; residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinate care with multiple providers across all levels and sites of care. Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluate care and outcomes to ensure timely and appropriate provision of services. Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrate performance and leadership that is broad enough to improve the care for a group of patients. Provide peers with formal or informal constructive feedback for safe, quality nursing care. Foster a safe and supportive environment conducive to the professional development of health care professionals. Evaluate case management outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented case management outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility. Practice as a Registered Nurse for the Montana VA Health Care System and provide direct support to the Community Nursing Home (CNH) program. Accountable for surveying the CNHs. Provide follow-up evaluations for VA patients that are placed in nursing homes. Review appropriateness for CNH placement. Represent Montana VA Health Care System with community contacts. Maintain positive and communicative relationships between the community, the Veterans and his/her family, and the VA. Execute position responsibilities that demonstrate leadership, experience, creative approaches to management of complex client care within the CNH. Collaborate with community agencies and VA staff in organizing and coordinate the clinical componets of care and request authorization of services and durable medical equipment in response to individual health care needs along the illness and care continuum and in multiple settings. Utilize all avenues and resources available to provide safe, efficient, cost effective use of healthcare resources and to foster patient self-managed care where possible. Possess intimate knowledge of federal regulations, VHA guidelines, medical review criteria, and utilization management process. Advance leadership and nursing skills are provided in a holistic approach. Assist with in-services, orientation of new staff and staff development. Responsible for care coordination and oversight activities that meet the individualized needs of the Veteran and national program requirements. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
