<p>The Community Care Women's Health Care Coordinator (RN) is responsible and accountable for all elements of the nursing process when providing, coordinating, and/or supervising patient care and Women's Health community consults. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care and the referral process.</p><h3>Major Duties</h3><p>Functions or Scope of Assigned Duties: The Community Care Women's Health Care Coordinator (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and education for Veterans and their spouses, including but not limited to coordinating and managing community maternity care, gyn care, infertility treatment, in vitro fertilization, mammography, cervical cancer screening, and bone density testing. Adheres to VHA Directives governing treatment eligibility and community referral eligibility. The Community Care Women's Health Care Coordinator (RN) assists Veterans, spouses, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care including both VA and community providers. Coordination also includes assuring services interface within the VA and community programs to provide continuity of care to Veterans. The Community Care Women's Health Care Coordinator (RN) provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care and ensuring medical records are received and scanned into the electronic medical record system. The Community Care Women's Health Care Coordinator (RN) tracks costs and attempts for approved IVF treatments. Tracks IVF cycles to ensure process is in accordance with VHA Directive 1334. Tracks intrauterine insemination attempts to ensure process is in accordance with VHA Directive 1332. Ensures authorizations for care are in place and that billing is accurate for in vitro fertilization. The Community Care Women's Health Care Coordinator (RN) coordinates and facilitates, as needed, women's health care including contacting the Veterans, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, retrieving medical records from the community and indexing them into the electronic health records, and notifying providers of needed follow up care. The Community Care Women's Health Care Coordinator (RN) will interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Anticipates and implements preventive patient care measures. The Community Care Women's Health Care Coordinator (RN) provides guidance and education to patients and caregivers and collaborates with other professionals at all levels. The Community Care Women's Health Care Coordinator (RN) addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. The Community Care Women's Health Care Coordinator (RN) ensures the plan of care and educational requirements are individualized and tailored specifically to support each individual patient's needs. Reports status of service needs and contributes to problem resolution and modification of processes as necessary. Supports the facility Women's Health Program and evaluates care and outcomes to ensure timely and appropriate provision of sex-specific services. The Community Care Women's Health Care Coordinator (RN) may be required to perform other assigned duties within or outside of the department as directed by supervision, to include direct patient care duties. Supervisory Controls: The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Chief of Staff Dimensions of Nursing Practice: Leads professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to improve evidence-based and/or data-driven outcomes for an identified patient population and/or specialized areas within immediate practice setting. Customer Services Requirements: Consistently communicates and treats customers (patients, visitors, volunteers, and Medical Center staff) in a courteous, tactful, and respectful manner.
