<p>The Community Care Registered Nurse (RN) functions as Clinical Coordinator, coordination care in conjunction with a team of clinical and administrative professionals. This RN coordinates and facilitates the Community Care program according to Veterans Health Administration standards and other accrediting bodies.</p><h3>Major Duties</h3><p>Major Duties include but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Initiates electronic progress notes to providers for all faxed requests for extension or additional services. Manages the clinical aspect of Community Care scheduling to include coordination of scheduling for services that can be provided with in the Veterans administration Health Care System with the Inter Facility Nurse Coordinator. Obtains clinical information on Veterans at local hospitals and documents this in the Patient record medical chart. Coordinates bed flow when Veteran is stable for transfer back to the VA Medical Center. Ensures medical documentation is received for care coordination and complete consults in a timely manner Coordinates community Care post-discharge needs, including prosthetics, home health, etc., to ensure Veterans discharge from Community Care receive appropriate medical supplies.
