Registered Nurse - Community Care Coordinator (GEC) - Intermittent

<p>This Registered Nurse - Community Care Coordinator position is located in Geriatrics and Extended Care Service Line located at the Togus, Maine VA Medical Center Augusta, ME location. This position is Intermittent, with varying hours per week.</p><h3>Major Duties</h3><p>Included but are not limited to: Serves as an Intermittent RN. Responsible and accountable of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates the effectiveness of the Veterans Care. Serves to support delivery pf high quality, timely care to the veteran receiving services in the community. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Work Schedule: Intermittent, Monday - Friday Virtual: This is not a virtual position.
