ABOUT THIS ROLE
<p>Bring your nursing leadership to Community Care, where administrative, programmatic, clinical practice, and operational oversight come together in service to Veterans. This role works with Department of Veterans Affairs and community providers, develops interdisciplinary processes, evaluates program effectiveness, and supports reliable, accessible health care for Veterans, families, and caregivers at the Sioux Falls Veterans Affairs Health Care System.</p><h3>Major Duties</h3><p>At the Sioux Falls Veterans Affairs Health Care System, help lead coordinated care that improves access, strengthens quality and safety, and supports meaningful outcomes for Veterans across services and disciplines. The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. Major duties include, but are not limited to: Accountable for the administrative, programmatic, clinical practice, operational oversight, coordination, and leadership for the Community Care department. Provides direct and indirect supervision of staff involved in the provision of services related to Community Care coordination, referral care management, home and community based services, transitions of care, and associated administrative support functions. Provides oversight for compliance with performance requirements, regulatory and accrediting body, and Veterans Health Administration standards, including directives, handbooks, and field guidebook. In collaboration with the Medical Director of Community Care, designs and implements interdisciplinary processes and programs within the service and health care system. Develops strategies and evaluates effectiveness of the program to achieve optimum patient outcomes in the most efficient and cost-effective manner. Processes and authorizes care, processes requests for appeals, acts as the clinical interface with Department of Veterans Affairs and community providers, and ensures timely access to care. Serves individuals, groups, and programs that cross services and disciplines and influence the organizational mission and impact health care and outcomes of care. Assesses and recommends allocation of human and materiel resources, determines staffing levels using directives, handbooks, or other models, manages overtime, and plays a significant role in recruitment, retention, professional development, and succession planning of staff. Confirms competency assessments and education are completed annually, ensures demonstrated proficiency, and facilitates appropriate corrective action when necessary. Reviews pertinent data bases for areas of oversight, monitors performance measures for trends, develops actions for early intervention, formulates follow-up plans, and chairs and/or serves as an active member of appropriate committees. Telework: Limited ADHOC-May be authorize (Employee who Telework on episodic, or short-term, and have completed required telework training and telework agreement). Permanent Change of Station (PCS): Not authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
