ABOUT THIS ROLE

<p>The Clinical Resource Nurse (CRN) is a registered Nurse (RN) who manages & coordinates care of patients in Surgical Care Clinics & the facility at large. They are responsible for assessing, planning, implementing, & evaluating direct & indirect nursing care. The CRN is accountable for care, focused on patient transition throughout the continuum of care, patient & family education, patient self-management upon discharge, and supporting factors that impact customer satisfaction.</p><h3>Major Duties</h3><p>Major Duties include but are not limited to: Responsible & accountable for all elements of the nursing process when providing and/or supervising direct patient care. Administers medications and procedures per established policies and guidelines. Responsible for the documented outcomes at the program or service level. Shows leadership in improving patient care and helps to enhance staff education and professional development using evidence-based practice. Influences outcomes by collaborating with members of the interdisciplinary team. Utilizes proficient level knowledge and skills at the microsystem level to improve patient outcomes. Assesses and evaluates patient care processes for effectiveness; initiating and changing when indicated. Follows and monitors staff compliance with safe work practice and procedures and provides education and re-direction where needed. Access and direct staffing if needed in the assigned areas. Identify and promote process changes within the service line. Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting. Demonstrates the ability to improve the effectiveness of communications and interactions with others, to inspire their cooperation and confidence to ensure safe and effective outcomes in the current practice setting. Expands knowledge base and practice evaluation through mentoring, precepting, and/or teaching to improve outcomes in the immediate practice setting.
