<p>The position is for a Registered Nurse Community Living Center (CLC) Restorative Coordinator in the Geriatrics & Extended Care Service for VA Long Beach Healthcare System located in Long Beach, California.</p><h3>Major Duties</h3><p>The Registered Nurse Community Living Center (CLC) Restorative Coordinator Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates patients in the CLC upon admission to Restorative Care Program and monthly thereafter to determine the potential for the program. Responsible for screening Veterans and conducting initial assessment prior to admission to the Restorative Care Program. Outline and implement measurable functional goals for Veterans to reach their highest level of function and maximize their independence. Collaborates with educators, Occupational Therapy (OT), Physical Therapy (PT) in recruiting and admitting patients to the Restorative Care Program and develops goals to maximize the patient's independence and functioning. Evaluates patient progress toward their goals monthly and institutes changes in therapy, goals, and activity when necessary. Completes the restorative nursing sections of the Minimum Data Set (MDS), specifically the observation period, quarterly reassessments, and significant changes in care. Monitors assignments to ensure they are appropriate with the level of education and training demonstrated of restorative personnel. Acts as a role model, mentor, and coach to the program's staff. Provides developmental opportunities for program staff to acquire clinical restorative nursing skills and expertise. Accountable to develop and implement educational programs and/or in-services to keep nursing staff current in restorative care practice and documented competencies. Implements strategies and programs for restorative nursing care that is consistent with the facilities mission, vision, policies, goals, and objectives. Responsible for the outcomes of the patient/client as measured by the MDS quality indicators, which is shared with the multidisciplinary team. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Questions clinical practices for the purpose of providing evidence-based care. Preferred Experience: Recent RN experience providing geriatric, gerontology, palliative care, or care of older adults in settings such as Community Living Center, Rehabilitation Center or similar; Case Management or care coordination experience including use of clinical pathways, risk stratification tools, or integrated care models for medically and psychosocially complex patients; Working in an interdisciplinary team setting, collaborating with multiple disciplines (e.g., medicine, social work, pharmacy, rehabilitation, mental health) to develop and implement patient centered plans of care; Providing caregiver support, education, and coaching for families of Veterans with functional impairment, or multiple chronic conditions; Prior VA experience or familiarity with VA electronic health record (EHR) systems, consulting processes, and performance measures related to geriatrics and long term services and supports; and National certification in case management or care coordination (e.g., CCM, RNP Certification, Gerontological Nursing) is highly desirable.
