ABOUT THIS ROLE
<p>The Registered Nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling.</p><h3>Major Duties</h3><p>The Registered Nurse will perform assignments at the Community Based Outpatient Clinic (CBOC Hemet) as a member of the Patient Aligned Care Team (PACT). Registered Nurse is responsible and accountable for all elements of the nursing process when providing direct patient/resident/resident care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient/resident/resident transition through the continuum of care, patient/resident/resident and family education, patient/resident/resident self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Provides a full range of nursing care to patient/residents with complex care needs utilizing the nursing process based on the ANA Standards of Nursing Practice. Recognizes subtle changes in patient/resident's condition and responds timely, choosing the correct interventions. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient/resident care problems on the unit. Performs ongoing assessment of patient/resident condition and response to treatment. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. Initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the medical record patient/resident/resident findings, assessments, and care provided. Evaluates and documents patient/resident/resident response to interventions and progress towards attainment of goals and expected outcomes. Revises interdisciplinary plan of care based upon on-going patient/resident/resident assessment and evaluation. Uses the Computerized Patient/resident Record System (CPRS), pyxis and/or the Bar-Coded Medication Administration (BCMA) software if applicable per policy. Uses a team approach to identify, analyze, and resolve patient/resident care problems that result in improved patient/resident outcomes. Provides direction to staff for unit activities in meeting patient/resident care needs. Ensures that delegated tasks as assigned are completed by other staff. Ensures the physician and/or the nurse manager/ immediate supervisor/coordinator are notified of untoward events involving patient/residents and staff, either by assigned RN or by self. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. Preferred Experience: BSN 1-year RN acute care experience. 1-year RN PACT experience or outpatient clinic experience Work Schedule: Monday -Friday , 8:00am-4:30pm.
